The short answer
For most nurses and doctors who read this page, OET is the better pick, and not because it is easier English. The NMC, AHPRA and the GMC accept both tests at levels they treat as equivalent: Grade B in OET and Band 7 in IELTS Academic. OET costs more per attempt (AU$587 against INR 19,000 in India), runs on fewer dates, and its paper results take longer outside India and the Philippines. What it gives back is content you already live with: a referral letter instead of an essay on urban planning, a five-minute role-play with a patient instead of a cue card about a festival. One more fact decides it for many: the OET 2025 performance data shows that nurses from Kerala, the Philippines and North India already pass OET Speaking at rates above 84 per cent. The place they fall is Listening and Reading. So the honest question is not "which speaking test is easier" but "which Listening and Reading can I beat". The rest of this page answers it regulator by regulator, with a decision table at the end.
- Acceptance: NMC and AHPRA accept both; GMC accepts both (OET Medicine); ECFMG Pathways accept OET Medicine only. AHPRA takes OET on Paper or on Computer at a test centre, not OET@Home; the NMC and the GMC take all three modes.
- Target: NMC wants IELTS 7/7/7 with Writing 6.5, or OET B/B/B with Writing C+. AHPRA, for tests from 23 April 2026, wants OET L350 R360 W350 S360 or IELTS 7 overall with Writing 6.5.
- Fee: IELTS INR 19,000 (IDP India). OET AU$587 (OET help centre).
- Retake one skill: IELTS One Skill Retake INR 12,650 (IDP India), only after an IELTS on Computer test and within 60 days of it (ielts.org); approved by AHPRA, not accepted by the NMC. OET on Paper single sub-test AU$315; the NMC combines two OET sittings within 12 months, the GMC does not combine at all.
- Results: IELTS on Computer within 1 to 2 days and IELTS on Paper in 7 days (IDP India results page). OET on Computer as little as 48 hours; OET on Paper as little as 5 days in India and the Philippines, from 13 days elsewhere (OET booking page).
Who accepts which: NMC, AHPRA, GMC, ECFMG
Start here, because the regulator decides the test for some of you before any other factor counts. The table uses the regulator's own page where we could read it this week and says so where we could not.
| Regulator | IELTS Academic | OET | Two sittings? | Source status |
|---|---|---|---|---|
| NMC (UK nurses, midwives, nursing associates) | 7 in Listening, Reading, Speaking; 6.5 in Writing. All four skills at the same time; no One Skill Retake, no IELTS Online | Grade B (350) in Listening, Reading, Speaking; C+ (300) in Writing. Nursing version. Paper, Computer and OET@Home all accepted | Yes, within 12 months and both within two years of assessment; floors 6.5/6.5/6.5/6 (IELTS) or C+/C+/C+/C (OET) | NMC pages read 19 and 20 Oct 2026 |
| AHPRA (Australia, all registered professions) for tests from 23 April 2026 | Overall 7; 7 in Listening, Reading, Speaking; 6.5 in Writing | Listening 350, Reading 360, Writing 350, Speaking 360. Paper or Computer at a test centre only; OET@Home is not accepted | Yes, two sittings within 12 months. For tests from 23 April 2026 no component in either sitting may be below 320 Listening, 340 Reading, 350 Writing, 350 Speaking (OET), or below 6.5 in any IELTS component | AHPRA page read 19 and 20 Oct 2026 |
| GMC (UK doctors) | Academic, 7.5 overall and at least 7.0 in each skill, from the same test and your most recent sitting | OET Medicine, Grade B in all four, from the same test and your most recent sitting. Paper, Computer and OET@Home accepted | No for either test; certificates are valid for two years | GMC pages "Using your IELTS certificate" and "Using your OET certificate" read 20 Oct 2026 |
| ECFMG (US doctors, Pathways) | Not accepted for the Pathways communication skills requirement | OET Medicine: at least 350 in Listening, Reading and Speaking and 300 in Writing, in one test administration, taken on or after 1 January 2025 for the 2027 Pathways. OET@Home only if your country has no test venue | No: miss one sub-test and you retake all four | ECFMG Pathways OET page read 20 Oct 2026 |
| Others (NMBI Ireland, Nursing Council of New Zealand, DHA Dubai, CGFNS and US state boards) | Both tests appear on these regulators' lists, usually at IELTS 7 or OET B. The NMBI page (read 20 October 2026) asks for OET Nursing B in Listening, Reading and Speaking and C+ in Writing from one result, less than two years old, and does not accept OET@Home. The Nursing Council of New Zealand and TruMerit (CGFNS VisaScreen) pages were also read on 20 October 2026; we have not verified the DHA Dubai or US state board pages this week. Our regulator requirements page tracks them one by one | NMBI, New Zealand and TruMerit verified; DHA and state boards unverified | ||
Two things in that table matter more than they look. First, the NMC window is 12 months, not the six months that many coaching sites still quote; the NMC accepted-tests page states 12 months and adds that both tests must be completed no more than two years before your application is assessed. Second, AHPRA's OET bar is no longer a flat B: Reading and Speaking now need 360, which is one ten-point step above the B threshold. If your route is Australia, the number to train for is 360, not 350, and the mode to book is paper or computer at a venue, because AHPRA does not accept OET@Home; the NMC does.
Format side by side
The two tests take about the same time in the room. The difference is what the time is spent on.
| Sub-test | IELTS Academic (ielts.org) | OET (OET Test Handbook 2026) | What changes for you |
|---|---|---|---|
| Listening | About 30 minutes, four recordings, general and academic topics | About 40 minutes, three parts, 42 questions: consultation extracts, short workplace extracts, a presentation or interview | OET Part A is note-taking from a consultation at speed; it rewards ward experience and punishes slow writing |
| Reading | 60 minutes, three long academic passages | 60 minutes, three parts, 42 questions: Part A is 15 minutes of fast lookup across four short texts, B and C are workplace documents and two longer articles | OET Part A is a race; most candidates who miss B lose it here and in Part C |
| Writing | 60 minutes, two tasks: describe a chart, table or diagram, then an essay | 45 minutes, one letter (referral, discharge, transfer or advice) written from case notes: 5 minutes reading, 40 minutes writing | No essay. You select from notes you understand; the marks are for choosing what the reader needs |
| Speaking | 11 to 14 minutes, three parts with an examiner: interview, a two-minute talk from a cue card, a discussion | 20 minutes in total: a short warm-up conversation, then two role-plays, each with 3 minutes to prepare and 5 minutes with an interlocutor playing the patient or carer | You lead the conversation. You are marked on four language criteria and five clinical communication criteria |
| Total | 2 hours 45 minutes | About 2 hours 45 minutes across the four sub-tests | Same stamina required |
Profession matters in OET and not in IELTS. OET Listening and Reading are common to all twelve professions; Writing and Speaking are profession-specific, so a nurse writes a nursing letter and plays a nurse. The OET on Paper mode is open to seven professions (Medicine, Nursing, Dentistry, Pharmacy, Physiotherapy, Radiology and Veterinary Science, per the OET on Paper page; the booking page calls Radiology Radiography); the other five must book Computer or OET@Home. Full detail is on our format and modes page.
Which score equals which
OET reports each sub-test on a 0 to 500 scale in ten-point steps and maps the number to a grade from A to E (OET results page). Grade B starts at 350 and C+ at 300. IELTS reports bands from 0 to 9 in half steps. Regulators are the ones who set the equivalence, and they are consistent: the NMC pairs B with 7 and C+ with 6.5, and AHPRA pairs 7 with its 350 to 360 OET band. OET publishes its own comparison table on the results page; the regulator thresholds are what count for you.
| Purpose | IELTS Academic | OET grade (score) |
|---|---|---|
| NMC Listening, Reading, Speaking | 7.0 | B (350) |
| NMC Writing | 6.5 | C+ (300) |
| NMC floor in a combined sitting | 6.5 (Writing 6.0) | C+ (Writing C) |
| AHPRA from 23 April 2026 | 7.0 overall, 7/7/7, Writing 6.5 | L 350, R 360, W 350, S 360 |
| GMC doctors | 7.5 overall, 7.0 each, same test | OET Medicine B in all four, same test |
One practical difference: the OET results page says a candidate awarded Grade B in Listening or Reading "typically" scores at least 30 of 42 marks. That gives you a raw target you can check against the free sample tests on OET Ready. IELTS does not publish a raw-mark table for Band 7 in the same way, so OET is the easier test to self-diagnose. Our results and grades page has the full scale.
Cost, retakes and results time
Money is where IELTS wins outright on the first attempt and loses some of the lead on the second.
| Item | IELTS (IDP India, ielts.org) | OET (OET help centre and mode pages) |
|---|---|---|
| Full test fee | INR 19,000 Academic or General Training; INR 19,250 for UKVI | AU$587 in all three modes, charged in Australian dollars. At October 2026 exchange rates that was above INR 30,000; our fees page keeps the rupee conversion current |
| Retake one skill | One Skill Retake, INR 12,650 (IDP India fee page); only after a full IELTS on Computer test, within 60 days of it, once per original test, results in 3 to 5 days (ielts.org). The AHPRA accepted-tests page says the National Boards have approved it; the NMC IELTS page does not accept it, because you must be tested in all four skills at the same time | OET on Paper only: one sub-test AU$315, two AU$415, three AU$515, only after a previous full test, not offered in the USA (OET help centre). The NMC combines two OET sittings within 12 months and, unlike its IELTS page, does not rule out a single sub-test sitting; confirm with the NMC before you book one. Not accepted by the GMC, which wants all four grades from the same test |
| Dates | IELTS on Computer dates run through the month at IDP centres; the IDP India site lists them by city | Paper on set Saturdays: 7 November 2026 (bookings closed 14 October), then 21 November, 5 December and 19 December 2026; booking closes 24 days before each date, and the booking page lists 21 November and 19 December for Nursing and Medicine only. Computer and OET@Home around ten dates a month, booking closes 7 days before |
| Results | IELTS on Computer within 1 to 2 days, IELTS on Paper 7 days (IDP India results page) | Computer and OET@Home as little as 48 hours after the last sub-test, 95 per cent within 6 calendar days; Paper as little as 5 days in India and the Philippines and from 13 days elsewhere, per the OET booking page (the results page quotes 12 days generally and 6 for the Philippines, so treat all of these as indicative) |
| Re-mark | Enquiry on Results through IDP | Within 72 hours of results (OET results page) |
So a nurse in Kochi who sits OET on Paper, misses Reading by one grade and re-sits Reading alone pays AU$587 plus AU$315. The same nurse on IELTS pays INR 19,000 plus INR 12,650 for a One Skill Retake, but if her regulator is the NMC that retake does not count and she pays INR 19,000 again. Add up the route you are actually on, not the headline fee. Our fees and retake page keeps the rupee conversion current.
Which is easier, honestly
Nobody publishes IELTS pass rates by nationality in a form you can compare, so the honest comparison is OET's own data against the thresholds. OET publishes grade distributions by first language, nationality and profession each year. The 2025 demographic data PDF on the OET test statistics page gives these shares reaching Grade B or above (A plus B):
| First language or group (2025) | Listening B+ | Reading B+ | Writing B+ | Speaking B+ |
|---|---|---|---|---|
| Malayalam | 27.8% | 24.0% | 73.2% | 85.0% |
| Hindi | 58.2% | 48.9% | 73.1% | 89.1% |
| Tagalog/Filipino | 67.4% | 43.9% | 57.9% | 85.4% |
| Arabic | 66.1% | 69.7% | 73.1% | 91.1% |
| Yoruba | 59.2% | 56.8% | 68.3% | 90.6% |
| Indian nationals | 37.3% | 32.0% | 72.6% | 85.9% |
| Nursing (all candidates) | 32.3% | 24.9% | 63.2% | 82.2% |
| Medicine (all candidates) | 77.6% | 78.9% | 79.9% | 92.8% |
Read that table before you read any coaching advert. Speaking is not the gap. More than four in five nurses from every language group we serve reach B in OET Speaking. The gap is Listening and Reading, and for Malayalam speakers it is wide: roughly one in four reaches B in Reading, and the largest single group (41.1 per cent) lands on Grade C. The reason is not vocabulary. OET Reading Part A gives you 15 minutes to find facts across four texts, and OET Listening Part A asks you to complete notes while a consultation plays once. Both are speed tasks in a healthcare register. IELTS Reading is slower and more academic; some candidates find that easier, many find it worse. Doctors, by contrast, clear every OET sub-test at high rates, which is why most doctors on the GMC and ECFMG routes choose OET without much debate.
What this means for your choice
If you have worked on a ward in English, OET Listening and Reading reward you because the texts are your daily paperwork. If you have not worked yet, or you studied in Malayalam, Tamil or Hindi medium and your English reading is slow, the OET speed tasks are the risk and you should sit one free OET Ready sample under time before you pay AU$587. If you score under 30 of 42 in Reading, you have a Reading problem in either test, and OET at least tells you so.
The TalkDrill view, for what it is worth: pick the test whose Reading and Listening you can beat, then let the speaking look after itself with daily role-play practice. Our Listening and Reading page is written for exactly the candidates in the first row of that table.
Decision table: who should pick which
| Your situation | Pick | Why |
|---|---|---|
| Doctor applying to ECFMG Pathways (USA) | OET Medicine | IELTS is not an option for Pathways; one administration, 350/350/350/300, and OET@Home only if your country has no venue (ECFMG Pathways OET page) |
| Doctor applying to the GMC | OET Medicine, usually | B in one sitting is a lower bar than 7.5 overall in IELTS, and the role-plays are consultations you already run |
| Nurse heading for the NMC, working in a hospital now | OET | Both tests accepted; your ward English carries Listening and Reading; the NMC combines two OET sittings within 12 months (confirm a single sub-test re-sit with the NMC first) |
| Nurse heading for the NMC, final year or not yet working, slow reader | Sit a timed OET Ready Reading sample first | Under 30 of 42: train Reading for a month before choosing. Both tests are accepted, so the test should follow the skill, not the advert |
| Nurse heading for AHPRA | Either, aim at 360 in OET, never OET@Home | AHPRA's OET Reading and Speaking minimums are 360 from 23 April 2026 and it does not accept OET@Home; IELTS One Skill Retake within 60 days is accepted by AHPRA, which is a cheaper safety net than an OET re-sit |
| Need the score also for a visa, a university or a non-health employer | IELTS | OET is recognised by health regulators and some immigration departments; IELTS is recognised almost everywhere. Check the exact visa rule |
| Budget is the binding constraint and you can sit within 60 days of a first attempt | IELTS on Computer | INR 19,000 plus a possible INR 12,650 retake, if your regulator accepts One Skill Retake (AHPRA yes, NMC no) |
| Pharmacist, dietitian, physiotherapist, dentist | OET on Computer | Your profession has its own Writing and Speaking; OET on Paper is open only to seven professions, and OET@Home is offered only where your country has no venue, per the OET Test Handbook 2026 |
| Already hold IELTS 7/7/7 with Writing 6.0 | Re-sit IELTS, not OET | The NMC lets you combine two IELTS sittings within 12 months; switching tests resets everything |
If you already sat one of them
Three common positions, and what the rules say for each.
You have an IELTS result with one skill short
For the NMC, book a full IELTS again within 12 months of the first sitting and make sure no band in either sitting is below 6.5 (6.0 in Writing). The NMC does not accept One Skill Retake. For AHPRA, One Skill Retake is accepted when it is taken within 60 days of the original test, per the AHPRA accepted-tests page, and IDP India lists it at INR 12,650. Do not switch to OET to rescue one IELTS skill: the NMC combines sittings of the same test, not across tests.
You have an OET result with one sub-test short
For the NMC, OET on Paper lets you re-sit only that sub-test (AU$315), and the NMC OET page combines two sittings within 12 months as long as no sub-test in either sitting is below C+ (C in Writing); the page does not say that each sitting must contain all four sub-tests, but confirm that with the NMC before you book a single sub-test. For AHPRA, two sittings within 12 months can be combined as long as no component in either sitting falls below 320 in Listening, 340 in Reading and 350 in Writing and Speaking (tests from 23 April 2026), and the re-sit must be paper or computer at a venue, not OET@Home. For the GMC, combining is not allowed; the grades must come from the same test and your most recent sitting, so you sit all four again.
You hold an old result
OET's results page says validity is set by the recognising authority, not by OET. The NMC asks that both sittings be completed no more than two years before it assesses your application; the GMC says IELTS and OET certificates are valid for two years, with a possible extension if you have passed PLAB 2 recently; AHPRA and others publish their own windows. Treat two years as the working rule and check the regulator page the week you apply.
How to prepare for either
Whichever test you choose, the speaking preparation is the same work: short, daily, recorded, with feedback. The difference is what you record.
- OET: one role-play a day with the AI playing the patient, five-minute hard stop, then listen back against the nine criteria. Start on TalkDrill role-play practice and read the criteria page once so you know what a B sounds like. Our six-week plan gives Listening and Reading the hours the data says they need.
- IELTS: one cue card a day with a timed two-minute talk, then Part 3 follow-ups. Use IELTS Speaking practice and the IELTS Speaking guide.
- Both: ten minutes of listening to native-speed healthcare English every day. Nurses who reach B in OET Listening are the ones who stopped translating; the hospital conversations guide is a good place to start.
